Financial Information
In order to prevent misunderstandings regarding dental insurance, we wish our patients to know that all professional services rendered are charged directly to the patient, and the patients are personally responsible for payment of fees. All accounts are due and payable at time of services rendered, unless prior arrangements have been made. We do not render our services on the basis that insurance companies will pay our fees. As a courtesy we will process any insurance paperwork so that any insurance reimbursements can be sent directly to the patient.
For your convenience we accept Visa, Discover, American Express and MasterCard. If you have any problems or questions, please ask our staff. They are well informed and up-to-date. They can be reached by phone at 310-858-8919, or by fax at 310-858-9297. Our office location is 414 N. Camden Drive, Suite #1240, Beverly Hills, CA 90210. Please call if you have any questions or concerns regarding your initial visit.
So that we may assure you and other patients of uninterrupted treatment, it is necessary for all patients to accept and adhere to a definite arrangement of appointments and fees. Once an arrangement is made, please remember this time is reserved for you. At least 24 hours notice must be given if cancellation is necessary, otherwise cancellation charges will be made.
